Fill in what applies. There is no submit button, because nothing is sent anywhere: the details only appear on the sheet. Leave a field empty and the sheet prints a blank line for writing by hand.
Preparing for your blood test
Fasting: your doctor will tick one.

Also remember: no tea, coffee, chewing gum, smoking or exercise during a fast. Bring this sheet and your test request slip.
The problem it fixes
The duty doctor explains how to prepare for a test. The patient nods, goes home, and comes back the next morning after a cup of tea. The test is repeated, the slot is lost, and the same explanation is given again tomorrow to someone else.
The idea came from a printed bowel preparation chart we found in a patient's file, with a photo of the pack on it. It had saved the doctor, the duty medical officer and the patient a good ten minutes each.
What it does
- One page for each instruction, written in plain words and approved by the treating doctor before it goes live
- A fill-in panel on every sheet: dates from a calendar, times from a clock, and either-or choices as ticks, so nothing is typed wrong
- No submit button. Nothing the desk types is sent or stored anywhere, and the screen says so
- Any field left empty prints as a blank line, so the same sheet still works with a pen
- Prints cleanly on A4, with a QR code that opens the online version on the patient's phone
- A WhatsApp button that sends the link to the sheet
- Every sheet is a proper web page that Google can find, so a patient searching for how to prepare finds your clinic's version
- Consent, checklist and request forms can be built the same way, as a printable pack
How it runs day to day
The words for each sheet live in one file. Adding a new sheet means writing it, getting it approved by the doctor, and publishing. The front desk only ever sees the finished sheets, grouped by department, with a search box.
